The name is the warning label. Two ounces of 151 proof rum, three quarters of an ounce of pineapple juice and a splash of triple sec is not a cocktail so much as a dare with fruit juice in it. There is nothing here to hide behind.
The ratio is what makes it notable. Most overproof drinks bury the rum under three or four ounces of mixer, which is the whole point of a Hurricane or a Zombie. This one runs the other way, with barely enough pineapple to tint the glass. The juice is not diluting the rum, it is flavoring it.

Bartenders tip: Treat this as a sipper served small rather than something you knock back. If you want the drink the name promises without the trip to the emergency room, build it over crushed ice in a rocks glass instead, which gets you the same flavor with dilution working in your favor. Never light 151 on fire in a glass someone is holding. And if you find the pineapple is losing the fight, more juice fixes it better than more triple sec does, since the orange liqueur is only adding sugar at this volume.
